We are partnering with an innovative, growing healthcare organization expanding into New Mexico and seeking an experienced Nurse Practitioner to join its clinical team.

This is not a traditional clinic-based role. It is an opportunity to help shape a new model of care designed to support medically complex patients where they need it most: at home.

The ideal candidate brings experience in value-based care, primary care, complex care coordination, and virtual care, along with strong clinical judgment and a genuine interest in helping build and grow an innovative care model in a start-up environment.

The team brings together physicians, Nurse Practitioners, Registered Nurses, Licensed Clinical Social Workers, therapists, and other healthcare professionals to proactively identify and address patient needs.

Through telehealth, remote care management, and close coordination with patients, families, caregivers, health plans, and primary care providers, the team focuses on:

• Preventing avoidable complications

• Identifying changes in health early

• Coordinating care across settings

• Supporting patients through transitions of care

• Improving quality and clinical outcomes

• Helping patients remain safely and independently at home

The patient population may include Medicaid and Dual Eligible members, including individuals with:

• Complex chronic medical conditions

• Physical disabilities

• Traumatic brain injuries

• Intellectual and developmental disabilities

• Functional limitations

• Complex geriatric needs

The role is primarily virtual, allowing you to deliver care through telehealth and remote care technologies while collaborating closely with an interdisciplinary team.

What You’ll Do

As a Nurse Practitioner, you will play a key role in the clinical management and coordination of care for medically complex patients.

Responsibilities include:

• Provide clinical care and ongoing management through telehealth and remote care technologies.

• Manage medically complex Medicaid and Dual Eligible patient populations.

• Collaborate with physicians and an interdisciplinary care team to develop and execute individualized care plans.

• Build strong relationships with patients, families, caregivers, primary care providers, specialists, and health plan partners.

• Identify changes in patient condition and intervene early to help prevent unnecessary emergency department visits and hospitalizations.

• Support patients during hospital admissions, discharges, and transitions back home.

• Participate in advance care planning and goals-of-care conversations with patients and families.

• Participate in a shared clinical triage and support model.
