Occupational Therapist

We are seeking a compassionate and skilled Occupational Therapist (OT) to provide high-quality, patient-centered Occupational Therapy services in the home setting. The Occupational Therapist will assess patients’ functional and cognitive needs, develop individualized treatment plans, provide therapeutic interventions, and collaborate with patients, families, physicians, and the home health care team to promote independence and improve quality of life.

Key Responsibilities

• Conduct comprehensive Occupational Therapy assessments to determine patients’ functional and cognitive abilities.

• Develop short- and long-term goals focused on improving or restoring independence, mobility, safety, and physical/cognitive functioning.

• Implement individualized treatment plans in accordance with physician orders and Agency policies.

• Educate patients and family members on therapeutic exercises, self-care techniques, adaptive strategies, and activities designed to support independence.

• Train patients and caregivers in the appropriate use of orthotic and prosthetic equipment.

• Design and fabricate customized adaptive or assistive devices when appropriate and provide instruction for their safe and effective use.

• Perform functional and cognitive re-evaluations to monitor progress and determine when treatment plans should be modified.

• Educate patients and families on all applicable aspects of the overall treatment program.

• Maintain accurate clinical documentation, progress notes, and required paperwork in a timely manner.

• Communicate regularly with physicians regarding patient treatment, response, and progress, and provide recommendations for changes to the plan of care as needed.

• Coordinate outpatient Occupational Therapy services when services cannot be provided safely or effectively in the home.

• Provide consultation, education, and support to other members of the home health team regarding equipment, techniques, modalities, and activity programs.

• Prepare required reports for physicians and third-party payers regarding patient care and progress.

• Maintain professional competency through continuing education, in-service training, and other professional development opportunities.

• Participate in staff meetings, case conferences, in-services, and Agency development activities.

• Perform other duties as assigned by the supervisor.

Schedule & Caseload

• Monday–Friday, 8:00 AM–5:00 PM

• One weekend per month

• Two holidays per year

• Typically 5–6 patient visits per day

• Local travel within the service area is required.
